How to request records
Neighborhood Health has partnered with HealthMark Group to ensure the accurate and timely completion of medical record requests.
Requests may be submitted electronically through HealthMark’s Request Manager at https://requestmanager.healthmark-group.com. Once logged in, select “Submit Request” from the menu options and complete all required fields to authorize HealthMark to process your request. You will receive a notification by mail or email once your medical records are complete and available for download.
